Provide therapeutic behavioral intervention, including assessment, diagnosis, and treatment, to youth with school, behavior, and developmental problems and interfacing with other health and human service provider agencies and individuals to promote patient care and community support.

 

The starting salary range for this position is between $72,000 - $78,000 annually.

Responsibilities

  • Provide direct social work and behavioral health/clinical services to children and adolescents involved in the GOALS program or receiving pediatric health care services. Behavioral health services will include psychosocial assessment, diagnosis, and therapeutic intervention (individual, family & group therapy).
  • Act as a liaison between the clinic and patient families, schools, and other community agencies.
  • Provide limited case management services to help facilitate appropriate school placement and to ensure patient is able to access any needed support services in order to optimize patient’s educational, emotional, and behavioral functioning.
  • Actively participate as a member of the interdisciplinary GOALS and pediatric teams.
  • Provide up-to-date referral sources, behavioral health expertise, and other relevant information and training to medical providers and other members of the treatment team.
  • Coordinate services provided by consulting psychiatrist and psychologists.
  • Provide education about puberty and other relevant health/mental health issues to PCC clients and their families.
  • Participate in program evaluation and planning activities as relevant to position and team.
  • Assist in developing and documenting PCC’s pre-adolescent service delivery model and procedures.
  • Identify opportunities to improve workflow, work processes, and patient care and to work cooperatively to implement those improvements.
  • Adhere to HIPAA guidelines.

Qualifications

Education:  Master’s degree in social work

 

Certificate/License:  LCSW required

Experience:  Minimum of two years’ experience as a social worker or counselor, preferably in a community-based setting. Must have experience working with children, adolescents, young adults and their families.

Knowledge, Skills, and Abilities:

  • Bilingual English/Spanish preferred.
  • Knowledge of clinical diagnoses and interventions.
  • Reflect values and professionalism of given discipline.
  • Excellent oral and written communication skills.
  • Knowledge of child and adolescent development and familiarity with community resources.
  • Organizational skills and the ability to manage multiple priorities in a fast-paced environment.
  • Ability to work independently with minimal supervision.
  • Ability to interact successfully with people from a variety of backgrounds and abilities.
  • Able to work effectively and cooperatively as a member of a team and with other clinic staff;
  • Ability to communicate effectively with persons of varying ages and cultural and socioeconomic backgrounds;
