Health Education

 

Health Educators support the mission of PCC by providing excellent patient support, education, and advocacy in all areas of health that directly contribute to critical clinic outcomes.

 

The starting hourly range for this position is $18.00 - $22.00.

 

Responsibilities

This position works as a member of a care team to coordinate patient care. They support patients and families to empower self-management, self-efficacy, and behavior change. Care team members are expected to assess and manage the health needs of the patient population and to communicate effectively with all patients, especially vulnerable populations.

 

ESSENTIAL FUNCTIONS: Under the direction of the Health Education Supervisor, this individual’s primary responsibilities include:

Perform direct patient education and advocacy:

  • Use motivational-interviewing to learn about patients’ needs and utilize the process of change to identify areas for improvement.
  • Provide comprehensive education and accurate information regarding methods of contraception, sexually transmitted infections, maternal and child health, and general wellness.
  • Collaborate with healthcare providers to identify patient needs and create individualized education plans.
  • Conduct individual and group education sessions to improve patient knowledge and awareness of health topics.
  • Evaluate the effectiveness of educational programs and modify as necessary to ensure patient engagement and understanding.
  • Maintain accurate and up-to-date records of patient education sessions and progress. - Participate in community outreach and health promotion activities to increase awareness of health-related topics.

Coordinate patient care and follow-up:

  • Provide information about medical services, social services, and referrals
  • Answer questions in a non-biased, non-judgmental, and non-directive manner.
  • Maintain organized patient caseload through implemented systems.
  • Notify clinician of special concerns pertinent to client’s visit and provide referrals to different departments depending on needs of client such as social work and community advocacy.
  • Identify opportunities to improve workflow, work processes, and patient care while working cooperatively to implement these and other quality improvements.

 

Implement education materials that meet the needs of specific target audiences:

  • Utilize health education resources
  • Prepare materials for patient distribution.

 

OTHER DUTIES AND RESPONSIBILITIES:

  • Comply with established risk management and safety procedures.
  • Understand and comply with regulations regarding Patient Rights.
  • Understand and comply with regulations regarding child abuse identification and reporting.
  • Understand and comply with regulations regarding infection control protocols.
  • Understand and comply with regulations regarding OSHA standards.
  • Maintain certification for Basic Life Support
  • Perform other duties as assigned.
  • Adhere to HIPAA guidelines.

Qualifications

Education: 

  • Minimum: High school diploma or GED.
  • Preferred: Associates or Bachelor’s Degree in a health-related field

Experience: 

  • Minimum of 1 year experience in a health clinic, family planning, HIV prevention, maternal health, child development, adolescent health, counseling, resource navigation, and/or experience working with health education.

Knowledge, Skills, and Abilities:

  • Extensive knowledge of prenatal and postpartum health and of family planning methods.
  • Ability to effectively present medical and health information related to physical health, child development, family planning, and pregnancy in order to improve health through prevention and maintenance.
  • Basic counseling and health education knowledge.
  • Ability to establish and maintain cordial and professional relationships with patients, volunteers, and clinic staff.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team. - Strong organizational and time management skills.
  • Ability to learn new health topics and teaching techniques and apply them to their work.
  • Ability to adapt to new processes with a positive attitude.
  • Ability to identify and utilize resources and referral systems.
  • Bilingual in English and Spanish required.
