Supports the mission of People's Community Clinic by coordinating the closing of population health care gaps, developing processes to reduce unnecessary ER and hospital visits, and achieving performance goals that improve patient care.

 

The starting pay range for this position is between $70,000 - $80,000 annually. 

 

Responsibilities

  • Champion organizational Population Health and Value-Based Care approach.
  • Identify and implement strategies to support care coordination efforts to close care gaps and improve patient outcomes.
  • Prioritize and coordinate recall lists and track outreach to ensure closure of care gaps, including appropriate screenings.
  • Develop and work with appropriate departments to implement processes to reduce unnecessary ER and hospital visits.
  • Oversight and revision of care pathways for patients at different risk stratification levels
  • Coordinate complex care conferences for high-risk patients.
  • Coordinate weekly meetings with the Population Health & Data Analytics Director and medical department heads to address pending follow-up items on higher-risk patients.
  • Work with the Population Health team to distribute data to the care team, including providers and nurses, regularly.
  • Work closely with health plans and be the principal liaison for all pay-for-performance and value-based care contracts.
  • Review health plan reports regularly and work with appropriate departments to implement population health improvement initiatives to improve performance and maximize value-based payments.
  • Identify opportunities to improve workflow and processes and work cooperatively to implement these and other performance improvement efforts.
  • Performs other duties assigned or as necessary
  • Adhere to HIPAA guidelines.

Qualifications

Education: 

  • Bachelor’s degree in Nursing

Experience: 

  • Two years’ experience in healthcare, including working in a clinical environment.
  • Experience participating in quality improvement and performance improvement efforts.

 

Knowledge, Skills, and Abilities:

  • Detail oriented
  • Strong Excel skills
  • Ability to work effectively with diverse individuals and populations
  • Excellent written and oral communication skills
  • Ability to work independently with minimal supervision

 

Certificate/License:  Current professional license to practice in the State of Texas.
