Overview

Dr. Appel with Patient

 

Supports the mission of People's Community Clinic by leading efforts to develop a population health program to include closing population health care gaps, coordinating implementation of risk stratification and panel-based care coordination, developing processes to reduce unnecessary ER and hospital visits, achieving quality performance goals that improve patient care, and managing and implementing value-based contracts.

 

The starting pay range for this position is between $90,000 - $100,000 annually. 

 

Responsibilities

  • Monitor and evaluate the strategic plan for sustaining an evidence-based population health model of care; Monitor and evaluate documented evidence based risk stratification and patient empanelment process to better align services with patient needs;
  • Monitor and evaluate care coordination to develop and revise care plans, follow, and close care gaps to improve health outcomes;
  • Oversee development of and execution on select clinical performance metrics, particularly those tied to pay-for-performance contracts;
  • Engage staff in quality improvement efforts that address specific clinical performance metrics to improve outcomes for those metrics;
  • Create and facilitate a set of complex care/chronic disease multi-disciplinary case conferences leading to improved outcomes on key metrics;
  • Create documented workflows and metrics related to ER and hospital utilization data with the goal of reducing unnecessary ER and hospital utilization rates;
  • Manage value-based contracts and guide/oversee value-based-related grants and activities;
  • Communicate with major payers (eg, health care plans) regarding current value-based contracts and identify opportunities for improvement in contracts and performance;
  • Serve as Project Lead for grants related to population health, including internal and external collaboration and grant-related reporting;
  • Work with other clinic departments to finalize contracts for consultants as needed;
  • Perform other related duties as assigned.

Qualifications

Education: 

  • BSN, MHI, MPH, or equivalent degree in healthcare field preferred.

Experience:

  • Minimum 3 years’ experience in health care leadership activities, including identifying and addressing care gaps in the patient population, especially in communities with chronic illnesses and diseases. 
  • Experience employing continuous quality improvement processes such as PDSA, Lean, or Six Sigma
  • Experience managing value-based contracts and/or programs
  • Experience managing diverse teams and leading without direct supervisory authority
  • Experience with program management

 

Knowledge, Skills, and Abilities:

  • Understand the current health care industry and value-based care
  • Familiarity with preventative strategies, population health, and quality of care frameworks
  • Familiarity with electronic health records and other healthcare IT systems
  • Highly organized and self-motivated with the ability to work autonomously
  • Collaborative working style with the ability to work with staff across different departments, teams and roles
  • Strong background in data collection and management, analytics and reporting
  • Strong verbal and written communication skills
  • Proven problem solving ability and able to juggle multiple priorities
  • Strong program management skills
  • Ability to interact successfully with people from a variety of backgrounds and abilities
