Assist the Chief Medical Officer in their duties and lead the Integrated Behavioral and social services department in a manner that Supports the mission of People's Community Clinic by overseeing therapeutic and social services to PCC patients and facilitating outreach efforts to improve service delivery in the community.

 

The starting range for this position is $95,000 - $105,000

 

Responsibilities

Lead and manage PCC’s Integrated Behavioral Health Program:

  • Have primary oversight of and responsibility for the IBH Department, including clinical services and supervision, quality assurance, departmental scope and planning, departmental metrics, policies and procedures, etc.
  • Work with the Chief Medical Officer (CMO) and Chief Operating Officer (COO) to set productivity goals for the department and develop realistic plans to achieve those goals
  •  
  • Provide clinical supervision for LMSW staff seeking LCSW licensure and ongoing supervision of LCSWs and IBH Program Managers.
  • Participate as available in cross-departmental comprehensive case staffings.
  • Serve as Social Service/Behavioral Health Representative on the Peer Review Committee
  • Participate in the Clinic’s Research & Ethics Committee.
  • Assesses capacity within and across departments and coordinates relevant training opportunities to increase staff skill and comfort in working with mental health.
  • Coordinates changes in workflows and procedures within the IBH Team and as they relate to other departments. Helps to communicate these changes to the team and medical staff.
  • Assists in data collection and manages reports for internal review and to identify and address potential growth areas.
  • Work with the Departmental Operations Coordinator to help support team productivity goals and give input for template development.
  • Collect data and prepare reports as required by funding sources and PCC management.

Provide Direct Services:

  • Therapeutic Intervention and Social Services to patients experiencing mental health issues.
  • Conduct mental health screenings and onsite consultations.
  • Complete intake assessments, including the client’s psychosocial history, current needs, diagnosis, and treatment recommendations.
  • Provide brief crisis assessments, psychoeducation, referrals, and/or interventions to patients during their medical visits, either in-room or virtually.
  • Provide in-office and virtual intakes, counseling, and brief crisis interventions to patients referred for depression, anxiety, trauma, etc.
  • Engage in collaboration among cross-functional teams to provide coordinated trauma-informed and person-centered care to patients with complex biopsychosocial needs to connect them to appropriate resources
  • Document services provided to clients per guidelines and policies.
  • Provide follow-up to patients to assess suitability of and follow through with referrals.
  • Document referrals and rates of client follow-through.
  • Identify opportunities to improve workflow, work processes, and patient care, and work cooperatively to implement these and other quality improvements.
  • Adhere to HIPAA guidelines.

 

Tandem Teen Prenatal & Parenting Program:

  • Serve as Project Coordinator for the Tandem Teen Prenatal & Parenting Program, which includes ongoing program development and oversight; coordination of interagency program staff and services to meet program objectives; facilitation of program staffing and management meetings, funding source development and maintenance; coordinate preparation of program grants, materials, and reports with Development team.
  • Maintain Tandem database and current client lists.
  • Supervise Tandem case management services as per program guidelines. Provide direct supervision to the PCC Tandem case managers.

Community Collaboration and Referral Linkage:

  • Communicate regularly with other service providers and referral sources to ensure current information and to facilitate client access.
  • Represent PCC at appropriate interagency forums and meetings.
  • Communicate regularly with other service providers, schools, and referral sources to provide referral and information updates to other PCC staff (medical, health education, and admissions) as needed.
  • Develop professional relationships with referral sources.
  • Develop and implement outreach and collaborative strategies as needed.
  • Maintain current knowledge of behavioral healthcare issues; build community collaborations; social work skills and practices; and social services and public health issues related to clients receiving services.

Qualifications

Education: 

  • Master’s Degree in Social Work

 

Experience: 

  • Five years of experience as a clinical social worker is required, preferably in a hospital, community health center, or other multidisciplinary setting.
  • Five years of supervisory experience is required.
  • Grant-writing and management experience required.
  • Clinical experience working directly with individuals and their families.

 

Certificate/License: 

  • LCSW-S required.

 

Knowledge, Skills, and Abilities:  

  • Ability to assess psychosocial and basic life needs of clients; crisis intervention management; case management; interviewing and supportive counseling skills; ability to assess the appropriateness of and to provide short-term therapy;
  • Strong written and verbal communication skills; patient documentation; principles of patient education; group facilitation; knowledge of community resources; program development; teaching/mentoring; knowledge of human development; Knowledge and experience providing direct client services, creating and maintaining referral linkages and effective community collaborations.
  • Ability to work effectively and cooperatively as a member of a team and with other clinic staff; ability to communicate effectively with persons of varying ages and cultural and socioeconomic backgrounds; effective networking skills with area resources and agencies; flexibility; initiative; dependability and leadership; time management; and organizational skills.
