The Health Educator supports the mission of PCC by providing excellent patient support, education, and advocacy in all areas of health with a focus on sexual and reproductive health and well-being. This position works as a member of a care team to coordinate patient care. They support the case management of patients and families to empower self-management, self-efficacy, and behavior change.
The starting rate for this position is between $22 - $24 hourly.
Coordinate patient care and follow-up:
Perform direct patient education and advocacy:
Implement education materials that are designed to meet the needs of specific target audiences:
Participate in Community Outreach events and partnerships to educate the community on reproductive health and well-being and HIV and STI prevention.
