The Health Educator supports the mission of PCC by providing excellent patient support, education, and advocacy in all areas of health with a focus on sexual and reproductive health and well-being. This position works as a member of a care team to coordinate patient care. They support the case management of patients and families to empower self-management, self-efficacy, and behavior change.

 

The starting rate for this position is between $22 - $24 hourly. 

Responsibilities

Coordinate patient care and follow-up:

  • Provide information about medical services, referrals, and answer questions in a non-biased, non-judgmental manner.
  • Be aware of patient’s other unmet health and social needs and work with appropriate PCC staff
  • Use case management to keep track of patients who have tested positive for an STI in order to reach out and educate them on medication usage, STI prevention, and PrEP counseling.
  • Keep track of patients who are uninsured and need PrEP medication. Assist them in filling out the application and meet with them if they have any future questions or concerns. Communicate with providers about patient’s status regarding PrEP and financial coverage of PrEP.
  • Notify clinician of special concerns pertinent to client’s visit to compliment the work of physicians and other specialists in the clinic.
  • Monitor adherence to care plans, evaluate effectiveness, monitor patient progress in a timely manner, and facilitate changes as needed.
  • Identify opportunities to improve workflow, work processes, and patient care while working cooperatively to implement these and other quality improvements.

Perform direct patient education and advocacy:

  • Use motivational-interviewing to learn about patients’ needs and utilize the process of change to identify areas for improvement.
  • Provide comprehensive education and accurate information regarding methods of contraception, sexually transmitted infections, and general reproductive health.
  • Maintain accurate and up-to-date records of patient education sessions and progress.  Participate in community outreach and health promotion activities to increase awareness of health-related topics.
  • Collaborate with healthcare providers to identify patient needs and create individualized education plans.
  • Conduct individual and group education sessions to improve patient knowledge and awareness of health topics.
  • Provide and explain appropriate forms to clients to ensure accurate completion and understanding, ensuring that clients with limited English proficiency receive information in the language they best understand.

 Implement education materials that are designed to meet the needs of specific target audiences:

  • Utilize health education resources
  • Prepare materials for patient distribution.
  • Maintain on-going professional development and current knowledge on trends in HIV, prevention and treatment, efforts to end the epidemic, and other related topics.

Participate in Community Outreach events and partnerships to educate the community on reproductive health and well-being and HIV and STI prevention.

  • Lead the development of program promotion and outreach/communications strategies to educate community on safe sex practices, reproductive health, and to increase community awareness and use of HIV prevention services.
  • Establish and maintain effective working relationships with staff, consultants, community leaders, key stakeholders, government agencies, and other local, state, and national agencies.

Organize training and guidance for other team members

  • Help train any new health educators, interns, or volunteers for the Department
  • Organize health education systems and processes so that they are clear and efficient for any other care team members to easily understand
  • Assist interns and volunteers with questions and guide them when needed with their tasks.

Qualifications

Education:

  • Associate’s degree or two year post-secondary education required.
  • Bachelor’s degree in public health or related field preferred.

Experience:

  • Minimum of 2 years’ experience in a health clinic, STI prevention, HIV education, child development, adolescent health, family planning, counseling, resource navigation, and/or experience working with health education.
  • Leadership and/or program management experience preferred.

 

Knowledge, Skills, and Abilities:

  • Fluency in English and Spanish preferred.
  • Understanding of human reproduction, family planning methods, human sexuality, health prevention, and wellness.
  • Knowledge of and sensitivity to the HIV/AIDS patient population and/or an eagerness to learn about this community.
  • Strong background in Sexual Health, STI and HIV Prevention, Health Equity, and experience working with populations of sexual and gender minority.
  • Ability to effectively present medical/health information related to physical health, child development, family planning, and pregnancy in order to affect health through prevention and maintenance.
  • Excellent communication and interpersonal skills.
  • Reflexive listening and strong skills in written/verbal communication.
  • Basic counseling and health education knowledge.
  • Ability to establish and maintain cordial and professional relationships with patients, volunteers, and clinic staff.
  • Ability to identify/ use resources and referral systems.
  • Ability to interact successfully with people from a variety of backgrounds and abilities.

 

Certificate/License: Basic life support certification
