The Community Health Worker (CHW) at People’s Community Clinic (PCC) plays a vital role in supporting our mission by serving a dedicated panel of patients and collaborating within an integrated care team. This position focuses on empowering individuals to achieve their preventive, chronic, and acute care goals, while identifying and addressing social and emotional barriers that impact their health. CHWs advocate for systemic change and connect patients with community-based resources to improve health outcomes and enhance quality of life.

 

The starting range for this position is $21- $23 hourly.

Responsibilities

  • Screening and Support: Identify and assess non-medical barriers to health, providing tailored support to help patients overcome these challenges.
  • Resource Education and Assistance: Educate patients on available community services and assist them with the application processes for programs related to food, housing, transportation, and other essential needs.
  • Referral Coordination: Utilize the FindHelp platform to refer patients and community members to appropriate resources, track referral outcomes, and follow up to ensure successful connection.
  • Case Management: Provide case management services, including home visits when necessary, to monitor progress, assess needs, and reinforce care plans. Maintain detailed records in the electronic health record (EHR).
  • Interdisciplinary Collaboration: Work alongside medical, behavioral health, and other care team members to develop comprehensive, patient-centered care plans based on insights into patients' daily challenges.
  • Clinic Support: Assist in the operation of PCC’s in-clinic resource programs, including the monthly food pantry, and contribute to other related initiatives.
  • Community Engagement: Participate in outreach events and health fairs to build trust within the community, promote PCC services, learn about available resources, and share relevant information with the CHW team.
  • Data Collection and Program Improvement: Collect and review data to identify emerging community health trends and contribute to program improvement efforts.

Qualifications

Education:  High School Diploma/GED

 

Experience:

  • Experience in a community-based, health-related, or social services setting preferred.
  • Demonstrated ability to support, motivate, and connect with diverse individuals.
  • Basic computer skills and comfort using online platforms for referrals and documentation.

 

Certification/License: 

Community Health Worker (CHW) certification preferred. If not currently certified, must be willing to complete CHW training and obtain certification within six months of hire. PCC will provide and fund the required training and certification.

 

Knowledge, Skills, and Abilities:

  • Strong commitment to community health and a thorough understanding of non-medical drivers of health.
  • Familiarity with public benefit programs and support systems (e.g., food assistance, housing, transportation).
  • Excellent interpersonal, communication, and motivational skills.
  • Culturally competent and respectful of diverse backgrounds, beliefs, and practices.
  • Highly organized, self-directed, and adaptable to changing circumstances.
  • Comfortable interacting with individuals at various organizational levels and within community settings.
  • Ability to handle emotionally challenging or stressful situations with professionalism.
  • Bilingual (English and Spanish or another language) is strongly preferred but not required.
  • Reliable transportation and insurance, as frequent travel for home visits and community events is required.
