Supports the mission of People's Community Clinic by providing therapeutic behavioral intervention and social services to high-risk patients and interfacing with other health and human service provider agencies and individuals to promote patient care and community support.
This position works as a member of an integrated care team to coordinate patient care and support patients and families in self-management, self-efficacy, and behavior change. Care team members are expected to assess and manage the mental health needs of the patient population and to communicate effectively with all patients, especially vulnerable populations.
The starting salary range for this position is $72,000 - $78,000 annually.
Under the supervision of the Behavioral Health and Social Services Director, this individual will:

OTHER DUTIES AND RESPONSIBILITIES:
Education: Master’s Degree in Social Work.
Experience: Prefer a minimum of two years’ experience as a social worker and/or therapist.
Knowledge, Skills, and Abilities:
Certificate/License: LCSW required
